SUMMARY:
We are currently seeking a Teen and Family Care Coordinator to join our team. This position is focused on populations of African American/African/African and Latine descent. This is a grant funded role currently funded through March 2026.
Purpose of this position: Under the general supervision of the Aqui Para Ti, Medical Director, and Primary Care Director, this grant-funded position is responsible for overseeing the Radical Healing Initiative. This position will develop and implement policies, protocols, and training methodologies to transform the Hennepin Healthcare System to provide comprehensive positive youth development and family-centered and culturally grounded care to youth and families of color.
The Radical Healing Program Coordinator will play an integral role in providing radical healing to Latine, African American, and Somali youth and parents. They will help coordinate grant-funded work to increase access to mental health care for youth and parents, coordinate services with MDH, support the new body mind soul group clinic and provider care coordination to 60 youth with mental health problems. The Radical Healing Program Coordinator will have the skills necessary to develop long-lasting relationships with key community organizations to foster collaboration for future community needs.
