Job Description

Pine Haven Care Center began its service to the elderly in 1964. We are a non-profit limited liability company that was organized in 1962 for the purpose of providing health care services for elderly and disabled persons. Pine Haven Care Center provides excellent Short-Term and Long-Term Skilled Nursing Care and Rehab Services, currently operates a 66 bed skilled nursing home.

Essential Functions:

 
Nursing Care Functions:
  • Ability to prioritize duties.
  • Is drug knowledgeable and reports resident response, side effects and reports problems or potential problems to the physicians.
  • Customer service oriented with knowledge of successful customer service techniques.
  • Clinical competence to utilize the nursing process to perform physical and cognitive assessments of residents.
  • Keep families updated or resident conditions
  • Must be a team player.
  • Role models, positive communication skills with NA/R’s, TMA’s, LPN’s, interdisciplinary team, residents and families.
  • Assist in developing methods for coordinating nursing services with other resident services to ensure the continuity of resident’s total regimen of care.
  • Participate in the development, maintenance and implementation of the facilities CQI/QA program.
  • Complete and investigate accident/incident reports and assist with abuse/neglect reporting to specific agencies as needed.
  • Acts as a liaison between physician, resident, family and resident care team to ensure physician orders are accurately carried out and documented using the information from the resident’s plan of care.
  • Listen to shift report and keep open lines of communication with supervisor, co-workers and on-coming shift.
  • Initiate and resolve follow-up and problem charting as needed.
  • Ensure wing personnel on your shift are carrying out their assignments.
  • Treat and ensure residents are treated with respect and dignity.
  • Follows shift routines, policy/procedures regarding monitoring of vital signs, weights, administration of medications and treatments.
 
Care Plan and Assessment Function:
 
  • Review new admission and transfers for appropriate assessment and documentation.
  • Assure completion of all necessary assessments required for the RAI process.
  • Participation in the ongoing review of the written plan of care for each resident for appropriate resident goals, problems, approaches and revisions based on nursing needs.
  • Assist in the monitoring and assessment of seriously ill residents as needed.
  • Determine and authorize with the interdisciplinary team the appropriate use of chemical and physical restraints/devices when necessary in accordance with facility policy.
  • Review nurse’s notes, NAR care sheets to ensure compliance with the resident plan of care.
  • Follow all DNR/DNI/CPR/comfort care protocols and orders.

 

Committee Functions:
 
 

  • Serve on, participate in and attend various committees and meetings of the facility to which you are assigned.

 
 
Personnel Functions:
 
 

  • Assists with the coordination of staffing patterns and productive nursing hours as delegated by the Director of Nursing.
  • Responsible for holding staff accountable for correctly following policies and procedures as outlined in the employee handbook and nursing job description.

 
Staff Development:
       
 

  • Assist the education director in identifying ongoing educational and training needs

Environment, Safety and Sanitation:
 
 

  • Follow up on all accident and incident reports and give to D.O.N. within facility time protocol.
  • Observes staff for proper safety techniques and infection control policies and procedures.$22.50/hour
  • Ensure the environment on the nursing unit is clean, safe, clutter-free and home-like.
  • Supervision of meal service to ensure quality of life with regards to the dining experience.
