Job Description
Overview:Fairview Range is looking for a Registered Nurse to join our International Falls Home Care and Hospice team. We consider it an honor to provide care for our patients and their families, and serve communities across the Iron Range. The Nurse Coordinator provides supervision to nursing and paraprofessional staff, assists the Branch Supervisor with management of daily operations. The Nurse Coordinator may also assist the Branch Supervisor with hiring, staff development, performance monitoring, etc.The Nurse Coordinator oversees the intake/admission process to assure that quality care is provided which is ethical, cost efficient, and therapeutic. This position assists in coordination of quality improvement activities as well as the coordination of the delivery of care and services appropriate to the needs of patients of all ages and their families using an interdisciplinary team approach. This position is full time, working 72-80 hours per two weeks, day shifts and every third weekend.Fairview Range is an affiliate of M Health Fairview, a partnership of Fairview Health Services, the University of Minnesota, and M Physicians. Collaborates with others as appropriate to assure admission is appropriate, reimbursement is maximized and that care needs can be adequately met.Develops a plan of care with specific goals for each client considering the physical and psychosocial assessment of the clients in collaboration with the physician.Provides ongoing case management of clients, which includes the provision of necessary education and support to the clients and caregivers, monitoring progress, providing nursing care and ensuring regulatory compliance.Contributes to employees ongoing awareness and support of total quality through education and role modeling.Qualifications:Required:One year of acute or long term care nursing experience.Graduate of an accredited RN program.Current RN Minnesota licensure required.Current BLS certification or attainment of certification within three months of employment.Current MN drivers license and insurance.Preferred:One year of homecare experience.One year of supervisory experience.Bachelors Degree in nursing.Knowledge of home care and hospice regulations.
