Job Description

Fairview Range is looking for a Registered Nurse to join our Hibbing Hospice team. We consider it an honor to provide care for our patients and their families, and serve communities across the Iron Range. The RN Case Manager coordinates the delivery of all the care/services appropriate to the needs of the patients of all ages and their families using an interdisciplinary team approach. An individualized plan of care with specific goals is developed for each patient in collaboration with the physician.

This position provides ongoing case management of patients, which includes the provision of necessary education and support to patients and their caregivers, monitoring progress towards goals, providing nursing care and managing reimbursement requirements. The RN Case Manager delegates duties to LPN's, and Home Health Aides, supervising them in the field according to policy; and collaborates with the Supervisor/Manager on issues related to case management, employee issues, etc. This position is 72-80 hours per two weeks, 8-hour Day shifts and rotating weekends and call.

  • Performs a Comprehensive Assessment according to established policies (including physical, psychosocial and spiritual data) upon admission, as well as ongoing established timeframes.
  • Assesses family and support systems active in the patient's place of residence.
  • Analyzes assessment data, identifies problems and develops the plan of care in collaboration with the patient, caregivers, and resource personnel.
  • Identifies expected outcomes that are realistic, measurable and attainable.
  • Collaborates with the interdisciplinary team to develop an appropriate discharge plan.

Required:

  • One year of acute or long term care nursing experience.
  • Graduate of an accredited RN program.
  • Current RN Minnesota licensure required.
  • Current BLS certification or attainment of certification within three months of employment.
  • Current MN drivers license and insurance.
  • Excellent organizational and documentation skills.

Preferred:

  • One year of homecare experience.
  • Experience with Windows based medical records software.
