Please visit childrensMN.org. Department Overview The Home Care and Hospice department at Children's provides care and support for patients who live in the Twin Cities seven-county metropolitan area. The team is made up of registered nurses, clinical social workers, child life specialists, chaplains, and intake coordinator who work collaboratively to provide care in the patients home. Services provided include home care and home care pharmacy, hospice care, palliative care, perinatal hospice program, and Children's Institute for Palliative Care. Position Summary Coordinates and provides family-centered nursing care to infants, children and adolescents in the home setting. Accountable for professional practice including independent nursing functions and delegated medical functions performed in collaboration with other health members. Responsible for continuous review of all aspects of every patient on his/her caseload to include: appropriate utilization of services; ensuring continued skilled need; monitoring of homebound status; review of documentation in the medical record; maintenance of interdisciplinary communication and discharge planning. Education/Certification: * Current Minnesota RN License, required. BSN, preferred. * Current BLS Certification, required. * Pediatric experience, required. * Home care experience, preferred. * Infusion experience, preferred. * Venipuncture skills, preferred. Requirements: * Must possess a valid driver's license (must meet minimum criteria of Children's Minnesota Fleet Safety Management policy). * Clean driving record. * Must possess and provide verification of current automobile insurance coverage.
