POSITION SUMMARY
-
The Chaplain incorporates the spiritual and emotional dimensions of human development into one's practice of care and reflects a working knowledge of different ethics theories appropriate to one's professional context. Additionally, the Chaplain is the spiritual care specialist of the care team responsible for assessing spiritual care needs and providing direct spiritual care and consultation to patients, families and staff. The Chaplain works with other members of the interdisciplinary team to identify and respond to spiritual and emotional needs and is attune to notice the relational dynamics and ethical principles at play in the clinical and professional setting.
This Chaplain position is a full time (1.0) position that serves several areas throughout the organization. Approximately 8 hours per week are dedicated to each of the following areas: Hospice (0.2), Care Center (0.2), Palliative Care (0.2), Hospital (0.2), and administrative responsibilities along with participation in employee support programs and community outreach with local ministerial services (0.2). The hours between departments may need to be flexed based on volumes and needs. -
POSITION QUALIFICATIONS

Education and Experience:
-
Masters or doctoral level degree in theology seminary by an accredited school required. National accreditation (ACPE or NACC) preferred.Experience working with death and dying, individuals/family/caregiver in a healthcare setting. 2-5 years experience as clergy or chaplain work in hospice or palliative care.

- Demonstrates ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of population served, including spiritual/religious informed care or service.
- Exercises skillful listening, compassionate presence & clear communication in one-on-one, small group, care conferences, and team meetings.
- Provide orientation and education for health care staff.
- Participate in regular meetings of the department and assigned clinical areas, including interdepartmental work.
- Provide triage/short-term support to members of the health care staff in support of their spiritual and emotional care, particularly in relationship to their care for patients and overall staff health. Collaborate with the employee assistance program (EAP) in conducting crisis debriefings and encourage staff to access EAP staff for self/family care needs.
- Knowledge of bio-medical ethics and ability to support ethical reflection and decision-making of patients, families and staff.
- On-site hospital and care center duties will include providing hospitality and support to: family members of critical trauma patients in the Emergency Department, dying patients and their family members, patients and their families for whom the Palliative Care Team has been consulted, Intensive Care Unit patients and families, and patients and families who have requested chaplaincy support.
- Skilled in providing effective spiritual care to those experiencing illness, injury, crisis, end of life, death, ethical dilemma, spiritual distress.
- Ability to triage and manage crises appropriately and provide spiritual care to persons experiencing loss and grief.
- Respect for and appreciation of diversity and differences including age, culture, language, sexual orientation, and religious/spiritual beliefs/practices.
- Knowledge of and ability to respond appropriately to the spiritual, ethical, and theological issues commonly encountered in the hospital setting.
- Ability to establish and maintain professional relationships with other members of the department, hospital staff, religious leaders and members of the community.
- Ability to provide religious/spiritual resources appropriate to the care of patients, families and staff.
- Knowledge and ability to educate other healthcare professionals about spirituality and religious practices.
- Self-motivated, able to function independently.
- Adheres to all departmental and hospital standards, policies, and procedures.

ESSENTIAL RESPONSIBILITIES
-
- Be self-reflective, including identifying one's professional strengths and limitations in the provision of care.
- Use one's own professional authority as a spiritual care provider through direct spiritual care for patients and families based on assessment of spiritual needs in clinical assignment.
- Advocate for patients and families.
- Triage and manage crises in the practice of spiritual care.
- Develop, coordinate, and facilitate public worship/spiritual practices appropriate to diverse settings and needs.
- Facilitate group processes, such as family meetings, post trauma, staff debriefing, and support groups.
- Provides/arranges for provision of sacraments/ rituals/prayers and integrative modalities at the request of patients and/or family members in a manner aligning with the traditions of the particular patient, family member, or significant other.
- Promote the integration of spiritual care into the life and service of the institution in which one functions.
- Promote, facilitate, and support ethical decision making.
- Foster a collaborative relationship with community clergy and faith group leaders.
- Recruits community chaplains, spiritual counselors, and spiritual care volunteers adequate to meet patient and family/caregiver needs by developing community contacts and offering education through congregations about hospice care.
- Assures spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner.
- Facilitates the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs.
- Provides patient centered spiritual counseling and support to patient and caregivers based on patient beliefs and desires.
- Maintains clinical records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision.
- Attends patient care conference and interdisciplinary group meetings as a member of the interdisciplinary group.
- Provides consultation, education, and support to the interdisciplinary group on spiritual care.
- Works with staff to provide bereavement to personnel, volunteers, and community clergy/spiritual counselors working with the hospice team.
- Assists in supervision of spiritual care volunteers when assigned to patients/ families/caregivers.
- Actively participates in quality assessment performance improvement teams and activities. Accepts and performs other related duties and responsibilities as required.
- Assures spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner.
- Plans periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team.
