FT (Sunday – Thursday or Monday - Friday, 9:00am – 5:30pm or 10:00AM – 7:30PM, on call every 8th week) Mental Health Practitioner/Case Manager position available at a Homeless Outreach Program in Minneapolis. This position will serve a caseload of approximately 50 clients with serious and persistent mental illness (SPMI) and dual disorders (MI/CD) that are homeless. The position will provide ACT services to the clients which includes: providing case management, teaching illness management and recovery skills, developing, directing and providing other treatment and support services. To qualify for this position you must have a bachelor’s degree in one of the behavioral sciences or a related field and have completed 2,000 hours of supervised experience in the service of persons with mental illness and some experience with the homeless population.
