FT (Monday - Friday, 4:00pm - 12:30am, on-call, weekend rotations) Mental Health Practitioner position available at a St. Paul in-home mental health crisis program. The position participates in admissions of recipients 24 hours a day, 365 days a year. The admission process includes a functional assessment, safety assessment, and provisional treatment plan upon which an individual Crisis Stabilization Treatment Plan is developed. The Mental Health Practitioner will provide brief, crisis-oriented counseling, ongoing clinical and psycho-social assessment for the duration of the recipient’s stay. The position will also require coordination of the recipient’s treatment and discharge planning with service providers such as the case manager, referrals to community resources, and advocating for the recipient as appropriate.

Requirements include a Master’s degree in Social Work, Psychology, or related field, a clean driving record and a reliable vehicle. Experience in the mental health field, community based services, crisis response or phone counseling preferred.
