Matrix Home Health Care Specialists was founded in 1987 by a forward thinking RN who recognized the need for health advocacy and care management for seniors. To meet the increasing need for high quality care in the home, Matrix began offering home health care services in 2006. Today Matrix is licensed in Minnesota to provide Comprehensive Home Health Care Services to seniors and persons with disabilities. Along with Care Management and Home Health Care, Matrix also operates a Residential Care Home for up to six residents in the south metro. 

Matrix is owned and operated by a hands-on RN and nationally certified geriatric care manager. Our primary focus is providing care that promotes independence, quality of life, and dignity for our clients. We use a team based approach to care giving in order to maintain a high level of continuity of care. While we are in the caring business we are also very much in the relationship business and nurses and caregivers are encouraged to and often form lasting relationships with clients and family members.

Our slogan is We're there for you and we truly are. Nurses and schedulers are available 24/7/365 to meet the needs of clients and caregivers. We also believe in the concept of the professional caregiver and in creating an environment that people want to work in.
